(Pursuant to Decision No. 291/ QD-UBND of the People's Committee of Dien Bien Province)
Step 1: The individual who applies for humanitarian medical examination and treatment shall submit the application to the Receiving and Notifying Department at the Office of Dien Bien Province Department of Health (Group No. 48, Residential Group No. 25, Muong Thanh Ward, Dien Bien Phu City, Dien Bien Province)
Step 2: Department of Health shall give the Application Receipt to the requesting individual as prescribed in Appendix 04 issued together with Circular No. 30/2014/TT-BYT.
If the application for permission to take humanitarian medical examination and treatment is incomplete, within 10 working days, there must be a written notice to the individual requesting humanitarian medical examination and treatment to complete the application, which must specify documents to be supplemented or amended.
Step 3: Within 10 days from the date of receipt of valid application stated on the Receipt, Department of Health shall issue an official dispatch permitting to take humanitarian medical examination and treatment.
If the humanitarian medical examination and treatment is not permitted, a written response must be provided, clearly stating the reason.
